Welfare and Benefit Fund Improvements

   <     New Smoking Cessation Benefits—Effective October 1, 2007

The Plan will now reimburse you for 50 percent (half) of the cost you pay for over-the-counter (OTC) aids, prescription medications, hypnosis therapy and laser treatments that promote smoking cessation. The Fund’s utilization review provider must first precertify any laser treatment you undergo. In addition, a doctor must prescribe all medications and treatments, including OTC aids, and you must fill your prescriptions at a retail pharmacy, not through mail order. The Plan will only reimburse for one attempt to quit, up to a maximum of $200 per individual, per lifetime. However, no deductible applies to this new benefit, which is available to all active employees and their spouses, retirees and their spouses, and surviving spouses.

 

<     Decreased Major Medical Deductible—Effective January 1, 2008

The major medical deductible is the amount you and your dependents pay each calendar year before the Plan begins to pay benefits toward major medical and retail prescription drug expenses. Effective January 1, 2008 , the Plan will implement a separate annual deductible for retail pharmacy benefits, and thereby reduce the major medical deductible as follows:

<     Improved Prescription Drug Pharmacy Benefit—Effective January 1, 2008

In light of the above change, your retail pharmacy benefit will no longer be covered under major medical or subject to its annual deductible. Instead, a separate annual deductible will apply as follows:

In addition, the Plan is replacing the current “cash and carry” retail pharmacy prescription drug program with a retail pharmacy prescription drug card program.

Under the retail pharmacy prescription drug card program, when you have your prescriptions filled at a Caremark participating pharmacy, you won’t have to file a claim form to be reimbursed for prescription drug expenses. The Plan will automatically pay 80 percent of the cost of the medication, after you have met the new retail pharmacy annual deductible. Here’s how it will work:

·          Go to any participating Caremark pharmacy.

·          Show your prescription drug ID card when you have your prescription filled.

·          Pay your 20 percent copayment for your prescription.

And that’s it!

Please note that when you have your prescription filled at a Caremark pharmacy, you will receive your medication at discounted prices. If you do not have your prescription filled at a participating pharmacy, you will have to pay the full cost of the prescription when you have it filled and then submit a claim for reimbursement from the Plan. The Plan will then reimburse you, based on the undiscounted cost, minus your copayment. This means that you will be paying more since you pay a percentage of a higher amount.

<      REMINDER: Adult Vaccination Benefits Available

Remember that the Plan will pay up to $150 per year for any vaccination you receive that is approved by the Food and Drug Administration (FDA). This includes flu shots, which can come in handy as flu season approaches. You do not need to meet a deductible and coinsurance provisions do not apply. This benefit is available to all active employees, retirees, spouses, and dependent children over age 18.

If there is a discrepancy between the wording here and the documents that establish the Plan, the document language will govern.
