Dear Member:

 

Effective February 1, 2007 the Plan will no longer be a part of Spectera’s Vision Network. We have entered into an  agreement with Vision Service Plan (VSP), a vision preferred provider organization (PPO). VSP was selected because of their large network of providers and their industry leading service standards. Providers that participate in the VSP network have agreed to charge discounted rates for services, which means less out-of-pocket charges for you. The enclosed VSP benefit summary is designed to give you additional information regarding VSP coverage.

 

When you go to an out-of-network provider, you may have to pay for the services at the time you receive them and then file a claim with Zenith Administrators (1-800-248-2141). Zenith will process all out-of-network claims according to the vision benefits provided for in the Plan Booklet. The benefits for an out-of-network provider are the same as the Plan has always had. The Plan pays 75% up to a maximum of $300.00 for covered vision care services. (Please refer to the Summary Plan Description for additional information on out-of-network benefits.)

 

It is always your decision whether to use network or out-of-network providers. Keep in mind that when you go to a VSP network provider:

 

•     You can have greater coverage and less out-of-pocket costs to pay.

•     Network providers file claims for you

•     You receive discounted prices on your vision care needs. This includes some services that are not covered by the Plan, such as cosmetic extras.

 

To locate a VSP network provider:

•    Ask your provider if he/she participates in the VSP network;

•    Visit their Web site at www.vsp.com; or Call VSP Member Services at 1-800-877-7195, Monday - Friday, 8:00 a.m. until 8:00 p.m.

•    Also enclosed is a VSP doctor listing for the State of Indiana. Please contact VSP if you would like a hardcopy listing of any other state.

 

Please read the enclosed benefit summary and keep it with your other benefits information. Also enclosed is a VSP ID card. If you do not have the card and services are received from a VSP doctor, be sure to identify yourself as a VSP member. (You can continue to use the blue sheet metal workers ID card for out-of-network services.)
